Try to import this survey and understand the settings used; It's in french but quiet easy to understand.
Basically:
1) Settings of your survey
=> field "send detailed email...": USE placeholder: {MAIL}
2) Set a radio button question with 2 answers option: A & B (as answer code)
3) create a hidden "equation" question with code: MAIL
