There is a pretty easy solution in Limesurvey which should do for you, but it is not 100% like in the image.

Look for "semantic differential". This will allow you to have one text to the left of the scale, and the other text to the right of the scale. You just need to separate the two texts with a "|" and everything to the left of the symbol will be shown left, everything after the pipe will be shown to the right of the scale. That should be even clearer for respondents than your paper example.
